Print the linked Washington Mutual New Account Worksheet.  Fill out form completely, checking “other”, for type of accounts.  Do not include credit card account numbers:  list type of credit card i.e. Visa, MasterCard, Discover, etc.  Send completed WM Worksheet along with a copy of your driver’s license to:

Washington Mutual will then send you a signature card and appropriate W9 in a pre-addressed envelope.  Please complete and return.  When your account has been successfully opened, you will receive your debit card.
